How can an individual receive a copy of a teaching certificate?

To get a copy of his/her teaching certificate, an individual must complete and mail to the Louisiana Department of Education a Request for Change/Action form, along with a personal check or money order in the amount of $10 and the completed Professional Conduct Form. All requested documents must be included. The application materials are included on the certification section of the State Department web site under Application Procedures and Downloadable Forms. You can obtain a free copy of your certificate by clicking the “Verify Teaching Certificate” link & locating your certification record online.
