How to Obtain Copies of Diplomas and Transcripts

Copies of diplomas and transcripts can be requested from local schools/school districts or the Lousiana Department of Education. Permanent records are under the control and care of local schools and school systems. Records maintained by the Department of Education are copies submitted by the local schools and school systems.

Request from Louisiana Department of Education - By Mail

To request a duplicate high school transcript or reissued high school diploma, the following steps should be followed. The volume of requests is unpredictable; therefore, we cannot guarantee the amount of time required for processing.

  1. Complete the request form (see below) and mail to the address indicated on the form. Be sure to include the correct year of graduation and the name of the high school.

    Send your application by CERTIFIED MAIL for receipt verification.

    Due to the volume of applications processed, the Department is unable to provide phone verification that your application has been received. If you need verification, please mail your application by way of CERTIFIED MAIL.
  2. Due to the confidential nature of the records, the graduate must authorize their release. If a third-party individual or agency is making the request on behalf of the graduate, attach to the request form a signed release statement from the graduate and a copy of the graduate's driver's license or other state-issued source of identification.
  3. Include the appropriate nonrefundable fees ($5 per transcript and $20 per diploma), and the acceptable forms of payments (money orders, cashier checks, and company checks made payable to LA Department of Education).

    ** Fee increase Effective September 1, 2009

    Effective September 1, 2009, fees were increased to $20 per diploma and $5 per transcript for all requests handled by the State.
  4. Because a reissued diploma is not valid until it is signed by the current local Superintendent, the School Board President, and by the local high school Principal, more processing time is required. All reissued diplomas will be routed directly to the local school system to obtain signatures. After signatures are obtained, diplomas will be forwarded to the address as indicated on the request form.

    Under no circumstances will the Department release an incomplete diploma.

    Important Notice: Students graduating from nonpublic schools who did not complete the state minimum requirements for graduation are not eligible for a reissued state diploma. These students will, however, be provided with a copy of the transcript submitted indicating their graduation status from the previously SBESE approved nonpublic school.

High School Graduation Verification - By Phone

To request a phone verification of high school graduation, please call 1-877-453-2721 (toll free). Phone verifications are conducted from 1:00 pm to 4:30 pm on weekdays.

Please have the following information available:

  • Student's Name (at the time of graduation)
  • Year of Graduation (and month if known)
  • Name of High School with location (District and City)
  • Date of birth

Errors on Transcripts
The Family Educational Rights to Privacy Act (FERPA) allows an educational agency to amend an education record if the record contains information that is inaccurate, misleading or an invasion of privacy. However, the Department of Education does not have the authority to change the information on documents submitted from local education agencies. This authority is granted to the local school system generating the high school transcript. Please contact the local school system from which you graduated for policies related to amending educational records.
